چکیده

Background: Hip joint preserving procedures delay the progression of femoral head osteonecrosis. Non-vascularised autologous bone grafts have been used to fill necrotic area and prevent collapse head. Harvesting use neck graft, minimizing donor site complications favoring regeneration after core decompression. Methods: 17 patients in early stage osteonecrosis were operated, 12 males 5 females with mean age 45.3 years old. Were treated decompression autograft harvested from neck. All evaluated an MRI preoperatively, at follow up X-rays hip functional clinical scores. Results: We didn’t observe any both up. returned previous everyday activities, free pain. There was a significant improvement regarding Conclusion: present surgical technique using for treatment The is simply sufficient most midterm cases adding advantage minimal invasing join

Treatment of Osteonecrosis of the Femoral Head by Core Decompression , Bone Grafting , and Electrical Stimulation

Core decompression is one of the more popular procedures for the treatment of early stages of avascular necrosis (AVN) of the femoral head. However, the controversy regarding its safety and effectiveness continues. In an attempt to clarify the situation, we are reporting on a large series of cases performed by a single surgeon with long-term follow-up. This is a prospective study in which resul...

Operative technique and results of core decompression and filling with bone grafts in the treatment of osteonecrosis of femoral head.

BACKGROUND The aim of this paper is to present the operative technique and results of core decompression and clearing of a necrotic zone in the femoral head followed by filling the defect with impacted autologous or synthetic bone grafts. This procedure allows for mechanical prevention of further collapse of the femoral head and loss of its spherical shape. It also creates conditions for the in...

Core decompression for early atraumatic osteonecrosis of the femoral head.

We performed 41 core decompressions in 32 patients for stage I or stage II osteonecrosis of the femoral head. The intra-osseous pressure at the intertrochanteric level was raised in 28 (68%) and there was histological confirmation of necrosis in 36 hips (88%). After a follow-up of 10 to 84 months (mean 31) nine of the 12 stage I hips (75%) showed significant clinical or radiological deteriorati...

Subchondral Insufficiency Fracture of the Femoral Head Treated with Core Decompression and Bone Void Filler Support

Subchondral insufficiency fracture of the femoral head (SIFFH) is characterized by acute onset hip pain without overt trauma. It appears as a low intensity band with bone marrow edema on T1-weighted MRI. The most common course of treatment is protected weight bearing for a period of several weeks. Total hip arthroplasty (THA) has been commonly used if the patient does not respond to the initial...

Core decompression with bone grafting for osteonecrosis of the femoral head.

Although core decompression is one of the more popular procedures for treating avascular necrosis, considerable controversy exists concerning its safety and effectiveness. The current authors review the results of a prospective study of 406 hips in 285 patients treated by one surgeon with core decompression and bone grafting. Patients were followed up for 2 to 14 years. The outcome was determin...
